Achieving Harmony
A 24-year-old with good oral hygiene visited our practice feeling self-conscious about her smile. She often smiles with her mouth closed, ashamed of her appearance when laughing. Despite having healthy, caries-free teeth, she wasn’t happy with the alignment and shape of her smile.

Intra-Oral Findings
Upon examination, several issues were identified:
- Angle Class 3
- Agenesis of Teeth 35 and 45: Persistent Milk Tooth (85), Extracted Tooth (75)
- Anterior Crossbite
- Retrusion and Distal Edge Rotation of Tooth 21
- Tooth Wear and Misshaped Teeth


Treatment Plan
To help her achieve the smile she desired, the following treatment plan was implemented:
- Alignment with Airsmile: We started with alignment therapy using Airsmile to correct her bite and improve the overall alignment of her teeth.
- Space Maintenance for Future Implant: We preserved space between teeth 34 and 36 for a future implant.
- Injection Technique for Misshaped Teeth: Using a flowable composite, we injected material into teeth 12-22 and 42-43 to reshape the front teeth and improve their appearance.
- Implant for Missing Tooth 35: An implant will be placed for the missing tooth 35 once the initial alignment and aesthetics are in place.




Outcome
After the successful alignment therapy, we focused on the smile design solution. The injection technique gave her front teeth a more natural, pleasing shape, and we provided night retainers to preserve the occlusion and alignment. The next step is to place the implant for tooth 35.
Though the ideal approach would have been to place the implant first and then reconstruct the canines for proper occlusion, the patient chose to prioritize aesthetics first. Moving forward, I will monitor her progress during regular check-ups and adjust the canines if necessary.
The patient is absolutely thrilled with her new smile and enjoys showing it off every day.

This case demonstrates the importance of tailoring treatment plans to individual needs and preferences, achieving both functional and aesthetic results. The patient’s happiness and confidence in her smile are the best outcomes of all.
